CONTACT LENS AGREEMENTS FOR TRADITIONAL "NON-DISPOSABLE" CONTACT
LENSES
The plans we offer allow you to replace
your currently prescribed lenses at a savings of approximately 40% off
replacement costs without an agreement. Replacements may be for any reason, including
damage, breakage, deposits, loss, spare pairs, or for power changes in your specific
lenses following your periodic contact lens examinations. The contact lens agreement cost
is only $30.00 for one full year. This fee includes a follow-up
exam with the doctor 2 weeks after new lenses are dispensed to insure proper fit
and optimum vision.
ADDED BENEFITS INCLUDE:
- Defective lens exchange at no charge within 30 days
of the date ordered
- 10% off eye wear, including prescription and
ophthalmic sunglasses
